Book layout for LSI - 71,000 words plus cover design a bonus!

I have a book going for proof reading next week and so will be ready for page layout in a couple of weeks. The book will be published by LSI I want the layout to look good! Ideally you can create a cover as well. I got very stressed with the first edition as there were numerous problems with the cover. The person did not understand LSI requirements.
