Header graphic for online resume

I am creating an online resume at   [obscured]   I need a very slick, professional looking header graphic 1000 x 288 pixels to replace the generic one that is on the site at the moment. To the left of the image I would like the text "Nathan Scrivener" and below it "Online Interactive Resume" To the right hand side of the image I would like to incorporate a photograph of myself, which I will supply. Everything else will be up to the winning contractor. Must help me portray a professional, mature, experienced image for a business-man.
